MEDICATION PROGRAM (Also known as PAP)
- Patients in need of long term prescriptions are to be enrolled in the Prescription Assistance Program (PAP).
PROGRAM POLICY CRITERIA
- Prior to PAP enrollment, prescriptions will be written for a two week time period, AT WHICH TIME, clients must return to GNC and see a physician for a new written prescription.
- Patients must follow through with PAP requirements, in order to receive FUTURE prescriptions.
- Requirements are a completed registration form, copy of a work-stub from self or supporting family member, and photo ID when possible.
- When applying for medications refills, allow three weeks's notification to PAP. This can be done by phone to GNC, or in person on Monday or Wednesday PM when clinic is open.
- Once enrolled in PAP, you must see clinic physician at least every 3-6 months, to continue receiving medications.
- Patients who do not follow through with requirements will not receive free medications at Good Neighbor Clinic's expense. The Good Neighbor Clinic pays for your medication at local pharmacies out of their public donations.
